Holly’s background as an emergency room nurse makes her uniquely suited to the role of Nurse Concierge, assessing patients, helping them understand their treatment plans and keeping their goals realistic.
As the Nurse Concierge at Our Clinic, I’m fortunate to be able to work with nearly every patient to learn their goals and help them get on a personalized treatment path.
New patients meet with me for a mini head-to-toe assessment and conversation, which can take up to an hour. There’s no cost to the patient for this assessment. During the assessment, I listen to the patient’s concerns and answer any questions they may have about the clinic.
Based on their concerns and a mini physical, I’m able to create a personalized treatment plan with the patient. I’ve found when patients are directly involved in the decision-making and planning of their care, they’re more motivated to participate in their treatment plan.
Many patients have told me they were nervous coming into the clinic for the first time. Their time with me, talking about their goals and concerns, helped them relax and be more comfortable. Because they’re more comfortable, patients often open up and talk about what’s truly important to them. Knowing the patient better allows us to create very personalized programs of treatment and keep their goals realistic.
I think getting up to an hour to have their questions answered is something pretty unusual in our healthcare system.
An important element of their meeting with me is giving the patient the tools to talk to their family doctor about their treatment plan. It’s good to keep the patient’s family doctor involved in their care so they’re able to monitor their successes.
When it’s necessary to have a prescription for their benefits to cover their treatment, patients get a letter to give their doctor outlining the plan. The doctor saves time by being able to write a single prescription for the treatment plan as the assessment has already been done and the results are provided to the doctor. For doctors with a heavy patient-load, this time saver can really help.
